To inquire about our service dogs or to request a preliminary application packet, please complete the following questionnaire

A little information recap before the questionnaire...

As previously stated on the Service Dog page

On average, there is a 12-24 month wait for a service dog.  We do not have any dogs "ready now", however, sometimes we have young dogs in training that may not be contracted yet.

I am sorry, there is not a set "menu" price for a service dog.

The price of a finished service dog is based on the dog, the type of tasks, the number of tasks, if any additional harnesses are needed, etc..

We will not train your pet to be a service dog.

You must be 21 years of age to move forward with a contract.  If you are between18-21, a parent must complete the contract if the application is approved.

We do not offer any financial assistance or grants. We do help our customers with their fundraisers they have going for their service dog in training.

We only train Seizure Detection and Seizure Response for Grand Mal / Tonic Clonic Seizures.

We do not train seeing eye dogs

You must be at least 18 years of age to complete the form below and request an application.
